WHAT IS A PATIENT NAVIGATOR AND WHY DO I NEED ONE? -An independent patient navigator is someone who works with you and for you creating your health plan.
Our healthcare system is complex and broken. Plagued with multiple difficulties, dysfunctions, and lack of coordination at all levels of care. An independent patient navigator works with you and for you through every step of the process. Whether you have multiple chronic conditions or have just been discharged from the hospital.
These services are not paid by insurance companies, hospitals, or health care systems. Therefore, I can put your interests first.
Professional patient advocacy services are a worthwhile investment. Now more than ever, it's important to be prepared.
While professional patient navigators have been sidelined from many face to face encounters, I will still be here working for you behind the scenes to ensure you receive the very best care.
I encourage people of all ages, sick or healthy, to create a basic medical profile in the event of health complications or emergencies. During these uncertain times, there are several discussions and/or preparations that will assist your treatment team in the event you are hospitalized.

RCS Health Profile
What information can and should be added to your health profile?
-Notable health changes that occur while you're at home
-Doctor or hospital visits
-Surgical procedures and any related complications
-Medications
Immunizations, like the flu vaccine or tetanus shot
-Special needs, such as blood sugar checks and seizure precautions
-Information that could aid staff in helping you with activities of daily living like bathing, dressing, swallowing or shopping
-Your doctor's contact information (name and phone number)
-Insurance information
-Pharmacy phone number
-Advance directives, a set of written instructions that specifies what actions are acceptable relative to a patient’s health if they are no longer able to make decisions due to illness or incapacity.
Alaska Advance Healthcare Directive
An advance healthcare directive, also known as living will, personal directive, advance directive, medical directive or advance decision, is a legal document in which a person specifies what actions should be taken for their health if they are no longer able to make decisions for themselves because of illness or incapacity.

The patient navigator services provided by Resolute Care Services, LLC are currently not covered by Medicare, Medicaid or any other insurance plans.
Payment for agreed upon services will be billed and provided as detailed in the Client Services Contract.
Services provided by Resolute Care Services, LLC do not include medical, financial, legal advice or recommendations.
Resolute Care Services, LLC does not provide medical diagnosis or any form of medical treatment. All clients must continue under the care and supervision of their licensed healthcare providers.
In addition, initiating communication via this website or by any other method (written, phone, email, voice message, text, etc.) does not constitute a client relationship with Resolute Care Services, LLC.
A signed Client Services Contract must be in place for said relationship to be initiated and prior to providing any and all services.

-Comprehensive personal and family medical history intake
-Comprehensive collection and analysis of medical records
-Medication reviews
-Creation of a health summary document containing key medical information to share with providers and family members
-Care coordination and communication
-Telephonic consultations (home, doctors office, hospital, emergency room, urgent care, senior living facilities, etc.)
-Assistance with determining realistic health goals
-Health literacy and learning needs assessment
-Healthcare education from credible and reliable sources
-Wellness coaching
-Assistance with Durable Medical Power of Attorney, Living Will/Advance Directives
Vetting providers and facilities
-End of term status evaluation, goal review, and identification of future healthcare needs
-Assistance with Medical Billing Review & Negotiation
Packages Includes:
-1:1 telephone consultation (up to 90 minutes)
-Gathering and organizing important healthcare related information and key contacts
-Basic medical history assessment and medication list review
-Creation of a Basic Health Profile (All digital with easy access/transmission for healthcare needs)
-Unlimited profile updates by email for the first year
-Quick conversion to "active status" should patient navigation services become necessary.
-HIPAA medical release forms for all providers
5 Hours Professional Patient Navigator Services
Fees will be collected upfront which includes 5 hours of patient navigator services.

Resolute Care Services, LLC charges $250.00 for a block of 5 hours of professional patient navigator services for contracted clients which can be used as needed.
10 Hours Professional Patient Navigator Services
Fees will be collected upfront which includes 10 hours of patient navigator services.
(see above for what's included in Professional services)
Resolute Care Services, LLC charges $500.00 for a block of 10 hours of professional advocacy services for contracted clients which can be used as needed.
